Here in the health office, our goal is to support physical, psychological, and emotional health to keep students in school and learning as much as possible! We work as a team with parents, teachers, and any other staff member to develop a plan to help best meet your child/ren’s health needs. On this page, you will find health information, various forms, and vaccine information. 



Help your child get off to a good healthy start to school by:

 

  1. Seeing your healthcare provider for routine scheduled check-ups. 
  2. Seeing your dentist twice a year.
  3. Seeing your eye doctor once a year.
  4. Ensuring they are up to date on immunizations
  5. Establish a good basic routine of sleep, nutrition and exercise.

  a. Sleep - 10-12 hours/night b. Nutrition – eat breakfast every morning, and maintain a healthy diet with at least 2 ½ cups of vegetables and 1½ cups of fruit every day. c. Exercise – encourage at least 1 hour of physical activity every day.

 

 


When does my student need to stay home? 

Individuals should stay home from school if they are displaying signs or symptoms of illness, including productive cough, shortness of breath, fever (temp of 100° F or higher), body aches, chills, headache, fatigue, sore throat, congestion or runny nose, nausea or vomiting, or diarrhea. If your student does not feel well enough to be in class all day and focusing well on school work, they should stay home. Students may come to school with minor symptoms of illness as long as they are feeling well enough to learn all day (i.e. stuffy nose, minor cold symptoms). 


The school does follow the Wisconsin Department of Health Services Childhood Communicable Disease Chart for exclusion criteria. 


Students are able to return to school when they have been fever free without the use of fever-reducing medication or have not had an episode of diarrhea or vomiting for 24 hours.  A student may return prior if there is a written statement from the provider stating that they are able to return to school.


If your child tests positive to any communicable disease please let us know at [email protected]. By keeping track, we can take extra sanitary precautions with deep cleaning to help reduce the spread of any unwanted germs. It also allows us to track illness in the classroom.



Medications at School

All medications must be kept in the health office. We are able to assist in dispensing any non-prescription or prescription FDA-approved medication during the school day. Detailed laws, protocols and procedures may be found on the Emergency Nursing Services document found in the district policies. Important things to highlight for both over-the-counter and prescription medications:


Over-the-Counter Medication: Any FDA approved over-the-counter medication may be administered at school with written parental permission. The form is below. You may also send an email to the health office but it must contain the following: Name of student, date of birth, current school year, name of medication, dose, route, time/frequency of administration, and reason it is to be administered. Any non-FDA approved medications or medications exceeding the therapeutic value must have a prescription medication form filled out by a Physician, NP, or PA. ALL MEDICATIONS ARE TO BE SUPPLIED BY PARENT/GUARDIAN AND MUST BE IN ORIGINAL MANUFACTURE PACKAGING. Any loose or expired medication will not be given. 


Over-the-Counter Medication


Prescription Medications: The health office must receive written permission from a Physician/NP/PA to administer any prescription medications. All medication must be brought to the health office by a parent/guardian. The medication must come in the original pharmacy labeled container. The label must be readable. No loose or expired medication will be accepted. All controlled medications will be counted with parent and staff members upon arrival. Pharmacy labels may act as a temporary provider order for short-term antibiotics only. The prescription medication form must be signed by the provider and parent/guardian.

Please fill out the student health information form if your child has any known or new health conditions. This information is utilized to help develop a health plan for your child if necessary. This information is shared only with staff who need to know to assist your child in school.

Please notify the health office right or download forms for you provider to fill out if your child has any of the following: 

Asthma

Seizures

Diabetes

Severe Allergies

Heart Condition

Or any other condition that may require an emergency care plan or procedure during school time

Vision or hearing concerns: The school conducts annual vision and hearing screenings on select grade levels. You may also request that a screening be conducted at any point throughout the year. Contact the health office with this request. 


Immunization Requirements: 

Wisconsin law requires students through grade 12 to be immunized according to their age/grade by the beginning of the school year. Parents/guardians are also able to sign a waiver for personal, religious, or medical reasons The most recent school vaccine requirements were updated in 2024. The form/waiver along with vaccine information sheets are available below.

When students do not meet the requirements of the law, parents are notified by legal notice. Failure to provide immunization records may result in exclusion. 


Helpful Hints:

  • Notify the school each time your child receives an immunization
  • Free immunizations are available through the Pierce County Health Department that are eligible. More information can be found through the VFC flyer or contacting the Pierce County Health Department

Incoming 5K Students

Going into kindergarten is an exciting time and we want to make sure your child is ready to kick it off with a healthy start! It is Wisconsin law that we must require immunization records. Immunization records and requirements can be found above. We also request that a health physical, dental exam, and eye exam form be completed and returned by the first day of school. Packets will be given to all incoming kindergarten schools with all the forms. Extra forms are attached below!

Pierce County Health


The School District of Spring Valley partners with Pierce County Public Health to provide onsite vaccinations. Pierce County comes to the school to deliver Flu vaccines to anyone student 18 years of age or younger with parental consent during the fall. The school will also host Pierce County Health to come to the school to do a Vaccine for Children Vaccine clinic 1-2 times a year. There are certain eligibility requirements, but this is a no cost clinic program offered to students age 18 and younger during the school day with parental consent. 


Eligibility Requirements

- Child is eligible for or enrolled in Medicaid/BadgerCare

- Child does not have health insurance

- Child has health insurance that does not cover vaccinations

- Child is American Indian or Alaskan Native
